Patient Registration at Leek Health Centre
If you’re unable to complete the online registration form, paper forms are available at our reception. Please bring two forms of identification (one photo ID and one proof of address), along with your NHS number if possible. If ID isn’t available, you can still register. Once registered, we’ll invite you for a New Patient Health Check.
Your Named GP
Every patient at Leek Health Centre has a named GP responsible for their overall care. For continuity, we encourage you to book appointments with your named GP, especially for ongoing health concerns. If your GP isn’t available, you’re welcome to see another doctor in the practice.
Temporary Residents
Staying in Leek or Staffordshire temporarily? You can register as a temporary resident for up to three months. Some visitors from outside the EU may incur charges for medical services. Please enquire at reception for more details.
Booking Appointments
You can pre-book appointments up to two weeks in advance. If you prefer to see a particular GP, please be aware that wait times may vary. We also offer telephone consultations for added convenience.
Home Visits
If you are too unwell to attend the surgery, please call us at 01538 381022 before 11:00 am to request a home visit. The duty doctor will call you back to discuss your needs.
Minor Surgery
We offer minor procedures at the practice, including joint injections. Please speak with your GP for more information.
Test Results
Test results are usually available within five days. To enquire, please call after 2:00 pm.
Repeat Prescriptions
To request a repeat prescription:
- Place the request form in the box in the waiting room.
- Email: [email protected]
- Ask your pharmacy to place the order.
- Use Patient Access to order online.
Prescriptions will be ready for collection after 3:00 pm, two working days later. We also offer the Electronic Prescription Service, allowing you to collect your medication from a pharmacy of your choice.
Updating Your Details
To ensure your records are kept up to date, please notify us of any change in your name or address. If you move out of the area, please register with a new practice promptly.
Research and Data Privacy
Leek Health Centre participates in healthcare research, contributing anonymous patient data to improve care quality and support medical research. Patient identities remain protected.
Chaperone Availability
If you require a chaperone for an intimate examination, please let us know in advance.
Car Parking and Accessibility
Patient parking is available at the rear of the building, with designated spaces for disabled patients near the entrance and in the car park. The car park is closed outside of practice hours. Our facilities include wheelchair access at both entrances, disabled toilets, and an induction loop hearing system for the hard of hearing.
